Physical Information
We can not accept physicals dated before April 1st of the prior school year.
Dickinson ISD Physical Paperwork
Dickinson ISD will be requiring new physicals for ALL students for the 2025-2026 school year.

Dickinson ISD has moved to a "paperless" physical packet for everything but the actual physical and medical history. This will help ensure accuracy in information as well as allowing parents to update information quickly and easily. You may download the medical history and physical forms by clicking HERE (en Español AQUÍ)
To begin, please have your child's name and DISD student ID number. Using a computer, tablet or mobile device, please log onto http://www.rankonesport.com/ to begin this process. You will select the option at the top "Parents Click Here" to begin, and follow the prompts from there to fill out the demographic and UIL paperwork for your child. If you do not have access to one of these methods to fill out the paperwork, please let us know and we will be happy to help you with an alternative.
You will be asked to provide information such as phone numbers, email addresses and other pertinent information. Please make sure this information is accurate and up to date as this information will be used in case of an emergency.
You will need to take the UIL Medical History and Physical forms to a Doctor of choice for your child's physical. This is the only paperwork that needs to be turned into the training room from this point on.
